Nomination Instructions

  • ​​​​​Before filling out yo​​ur nomination form(s), please review the award descriptions, nominator and nominee qualifications, and considerations in the table below.

  • If a nomination form does not open in the full-page view, please right click on the form link and choose the "open in new window" option.

  • You can stop and revisit any in​complete nomination forms at your con​venience. They are located at the bottom of this page.

  • Multiple nominators are required for all nomination forms, however only the user account that creates the nomination form will have editing rights. Please ​choose the primary nominator for your form with that in mind. 

  • An individual may not be self-nominated.

  • APHL staff are not eligible to nominate or receive an award - please do not ask them to submit a nomination on your behalf. However, APHL staff are available to assist you with confirming information about a nominee.

Nominee Limitations

  • ​​​​​Award winners are limited to 1 award per year. With exception to the Lifetime Achievement and Healthiest Laboratory Awards, current board members are not eligible to receive an award.

  • With exception to the Healthiest Laboratory Award, APHL awards should be conferred on an individual or laboratory only once. Laboratories are eligible to win the Healthiest Laboratory Award every three years.

​​​Decision Process

  • APHL's Ad Hoc Awards Vetting Commitee is formed from a diverse list of volunteers of your peers from APHL membership. The Awards Vetting Committee will review a​ll nominations; its recommendation will be considered and decided upon by APHL's Board of Directors.

  • Nominations are judged solely on the material submitted.​​
